Potential host galaxy: SDSS J154741.35+430853.5

Remarks: Possible supernova discovered by Roman Lisniak on the Blue DSS plate. The object is located 1" West and 4" South from the center of SDSS J154741.35+430853.5 = WISE J154741.34+430853.5 galaxy.
